AST: What Does This Enzyme Say About Your Liver and Heart?
AST (aspartate aminotransferase) is an enzyme found in your liver, heart, and muscles. When cells in these organs are damaged, the AST value in your blood may rise. The test is often requested alongside other liver enzymes. As you age, both the liver and heart may become more sensitive. Your doctor can evaluate the result in the context of your overall health. The information on this page is intended as general information and is not a substitute for professional medical advice. Always discuss your results with a doctor.
Reference Ranges
Reference ranges may vary between laboratories. When you order a test, a BIG-registered doctor assesses your personal results in context. For treatment decisions, discuss your results with your GP.
What It Measures
This test measures holotranscobalamin (holoTC), the complex of vitamin B12 bound to transcobalamin II. This represents roughly 10-30% of circulating B12 but is considered the only form available for cellular uptake and metabolic use.
Why It Matters
Older adults are more likely to develop B12 depletion due to reduced stomach acid and absorption capacity. Holotranscobalamin may detect this decline early, helping your healthcare provider intervene before neurological or cognitive symptoms develop.
When to Test
Testing may be recommended when total B12 results are borderline, if you have risk factors for B12 deficiency, or when your healthcare provider wants a more sensitive assessment of your B12 status. It can help clarify ambiguous total B12 results.

Lifestyle Tips
Supporting healthy B12 levels through diet may include consuming animal products such as meat, fish, eggs, and dairy. Those on plant-based diets should consider discussing B12 supplementation with their healthcare provider to maintain adequate stores.
